چکیده

PURPOSE To evaluate the outcome of autologous morselised bone grafting for a medial tibial defect of ≥10 mm deep in 45 total knee arthroplasties (TKAs). METHODS 38 patients aged 46 to 85 (mean, 73) years underwent 45 TKAs and autologous morselised bone grafting for a medial tibial defect of ≥10 (range, 10-25) mm in depth by a single surgeon. In the first 19 TKAs, 2 resected subchondral bone plates from the lateral tibial plateau were driven into 2 gutters made on the floor of the medial tibial defect as bony support posts. Morselised cancellous bone grafts were then impacted around these posts. In the remaining 26 TKAs, multiple drill holes were made on the floor of the defect to decrease the remodelling time. Morselised cancellous bone grafts were impacted to fill the defects. The tibial components were fixed with cemented (n=34) or non-cemented (n=11) technique. Internal fixation devices were not used; stem extension was used in only 2 TKAs (in a patient with Charcot joint and a patient undergoing revision TKA). RESULTS After a mean follow-up duration of 65 (range, 24-174) months, no patient showed gait disturbance. The mean knee flexion angle was 113.8º (range, 90º-130º). No absorption or collapse of the grafted bone, or component loosening were noted on radiographs. The grafted bone completely incorporated into the host bone, with bony trabeculae crossing the interface within 2 years in the first 19 TKAs and within one year in the latter 26 TKAs. The margin of the grafted area resembled bony cortex in 30 TKAs. CONCLUSION Autologous morselised bone grafting is a viable option for most medial tibial defects during TKA.

Union, Knee Alignment, and Clinical Outcomes of Patients Treated With Autologous Bone Grafting for Medial Tibial Defects in Primary Total Knee Arthroplasty.

Autologous bone grafting is an established method to overcome bone deficiencies in primary total knee arthroplasty (TKA). However, recently, metal augments have been used by many surgeons. Although autologous bone grafting is a common technique, few large studies have described its success in achieving bone union in primary TKA. The goal of this study was to evaluate primary TKA with autologous...

Autologous Bone Graft for Tibial Defects on Knee Total Arthroplasty

Osteoarthrosic knees with large bone defects usually develop as varus or valgus deformities, with daily functional disabilities such as pain, instability and limping. Between March 1996 and January 2003, 19 knees of 18 patients were treated through primary total knee arthroplasty using autologous bone graft using the Sculco modified technique to correct large tibial defects. Sixteen knees were ...

Long- Stem Total Knee Arthroplasty for Proximal Tibial Stress Fractures in the Elderly Patients

Background: Presentation of proximal tibia stress fracture is not infrequent among elderly patients due to their poorbone stock. Optimal management of patients with severe gonarthrosis of the knee and concurrent tibial stress fracture isnot known yet. In this study we report the outcome of primary total knee arthroplasty (TKA) using stemmed componentsin elderly patients. Methods: Between 2009 an...

Autologous Bone Grafting and Revision Plating in a Case of Persistent High Tibial Osteotomy Non-Union

INTRODUCTION Uni-compartmental knee arthritis may be treated with several surgical options including uni-compartmental knee arthroplasty, total knee arthroplasty and high tibial osteotomy. Non-union is a well-established and common complication that may arise from the latter option and few reports have shown successful treatment of persistent non-union of high tibial osteotomy. CASE PRESENTAT...

Does a Previous High Tibial Osteotomy (HTO) Influence the Long-term Function or Survival of a Total Knee Arthroplasty (TKA)?

High tibial osteotomy (HTO) is a well established technique for the treatment of medial osteoarthritis ofthe knee with varus malalignment. The outcome of total knee arthroplasty (TKA) after HTO remains uncertain. The aim of this paper is to revise the literature with the aim of answering the following question: Does a previous (HTO)influence the long-term function or survival of a TKA?. The sea...
